What are Alternative Payment Models and how do they impact primary care practices?

Alternative payments models (APM) and value-based payments (VBP) are terms that are used interchangeably and refer to a variety of payment types and are best defined for what they are not: open-ended fee-for-service payments, or straight pay for volume.
