Over 70% of patient visits to primary care practices involve one or more chronic conditions! Working collaboratively with patients to engage them in self-care is key to improving health outcomes. This module covers the essential components of self-management support (SMS) and provides tools to support SMS in your practice.
Learning Objectives
- Demonstrate the ability to coordinate care using an evidence-based personal care plan with goals prioritized by the patient.
- Demonstrate the ability to assist patients and/or families in connecting with peer support groups or other appropriate community resources.
- Demonstrate the use of motivational interviewing, readiness for change, the five As, and/or other appropriate communication skills with patients considering health behavior change.
- Demonstrate the ability to assist patients with developing effective action plans for health behavior change and other self-management activities.
