In Session 6, your practice will review your key successes and challenges for implementing SBIRT for your patients with UAU. Your practice will also develop a sustainability plan so that you can continue to improve the care of your patients with UAU.

Objectives

• Discuss the changes your practice has made over the past month to increase the utilization of team-based care for your patients with UAU.
• Discuss your practice’s key successes and challenges over the past 5 months for the implementation of SBIRT for your patients with UAU.
• Develop a UAU sustainability plan and a PDSA cycle.
