Helping patients by coordinating their care is a core feature of practice transformation and the Patient-Centered Medical Home. However, care coordination is one of the more challenging aspects of healthcare, as the U.S. has a fragmented system of care. This module provides examples of the value of care coordination and how staff and medical assistants can play a role in this important aspect of caring for patients.
