Staff: Care Coordination
What is care coordination and how can you, as a medical assistant or practice staff, be involved?
Helping patients by coordinating their care is a core feature of practice transformation and the Patient-Centered Medical Home. However, care coordination is one of the more challenging aspects of healthcare, as the U.S. has a fragmented system of care. This module provides examples of the value of care coordination and how staff and medical assistants can play a role in this important aspect of caring for patients.
After completing this module, the learner should be able to:
- Define care coordination in primary care practice.
- Discuss methods for coordination of patient care across the complex health care system, the practice, and the patient’s family and community.
- Describe the medical assistant role in care coordination.
Who should complete this module?
The information in this module is valuable for primary care staff.
