The rapidly changing healthcare environment requires innovation and transforming clinical practice to meet the quadruple aim:  better care, better patient experience, lower costs, and joy in practice.  This module introduces the learner to the key elements of practice transformation and the Patient-Centered Medical Home (PCMH), and the key role staff play in this process.
