What does it take to provide team-based care?

It takes a team to provide the care patients need every day, and building effective teams is central to practice transformation. This module will describe how to build effective team-based care, improve office efficiency, decrease health care costs, and the important role for medical assistants and staff.

Learning Objectives

  • Demonstrate the ability to practically apply a multi-disciplinary team approach to the care of patients.
  • Demonstrate the ability to participate as a team member in practice improvement, including evaluation of the practice and performance of PDSA cycles.
  • Demonstrate collaborative and respectful communication with other care team members during patient care and practice meetings.
