Value-based payments require primary care practices to rethink how they provide patient care, and this can feel overwhelming.
To address this concern, this module will discuss the key components for implementing value-based payment (VBP) models into your practice. These components include creating a mission and vision statements, addressing practice culture change, effectively utilizing your health information technology (HIT) for VBP, staffing, and payment models. Along the way, we explore examples of novel care delivery models.

- Describe the culture change considerations and approaches to implement successful VBP programs.
- Name potential methods to effectively utilize HIT for VBP implementation and operations.
