Welcome to the Clinician Practice Transformation (PT) modules! The objective of the Clinician PT modules is to provide a comprehensive and asynchronous learning environment for learners around the key components of practice transformation. The modules are designed to be interesting, engaging, and to enhance clinicians’ ability to participate fully in practice transformation activities. Please note that practice staff are encouraged to complete the Staff Practice Transformation modules.
