Welcome to the Practice Transformation e-Learning modules! These modules are offered through collaboration between the American Board of Family Medicine and the University of Colorado School of Medicine, Department of Family Medicine. Clinician Practice Transformation modules provide a comprehensive and asynchronous learning environment for learners around the key components of practice transformation. You have chosen to complete the Practice Transformation modules as part of your Knowledge Self-Assessment. In order to receive your Certificate of Completion for the Practice Transformation modules, you will need to complete all 11 modules and the evaluation.
