Welcome to the Practice Transformation (PT) modules! The objective of these modules is to provide a comprehensive and asynchronous learning environment for teams of learners around the key components of practice transformation. The modules are designed to be interesting, engaging, and to enhance teams’ ability to participate fully in practice transformation activities. Each module takes between 30-60 minutes to complete and includes videos, slide presentations, review of current research, and practice activities intended for practical implementation in everyday practice. Continuing Medical Education (CME) credits are available for completion of each module.
